33 year old male patient fisherman by occupation ,came to opd with
c/o headache since 1 year on and off and stiffness of joints since 3 months .
Patient was apparently asymptomatic 1 year ago , then he developed headache which is u/L right side ,dull aching pain .
Not associated with photophobia, phonophobia, watering of eyes, blurring of vision, vomitings. Since 3 months he developed stiffness of joints .not associated with restriction of movements,difficulty in standing from sitting position, fever, abdominal pain,cough, cold , diarrhoea.
Past history
N/k/c/o DM,HTN,CVA,CAD,TB,EPILEPSY
Examination
Patient is conscious, coherent, cooperative
No signs of pallor, icterus, cyanosis, clubbing, lymphadenopathy.
Vitals
Bp:120/80 mm hg
Pulse rate :86bpm
Temperature: afebrile
spo2 - 99
Systemic examination
CVS : s1,s2 positive,no murmurs
RS: BAE +,NVBS
CNS:NFND
P/A: soft, non tender .
Treatment
Tab naproxen 250mg PO/OD sos
Tab pantop 40mg PO/OD
